We are looking for a Strategic Discharge Networks Lead to oversee the expansion, management, and optimization of our Preferred Provider Network (PPN). This role will be pivotal in building and nurturing relationships with Skilled Nursing and LTC facilities, coordinating patient transitions, and ensuring a streamlined and data-backed transition process. The Strategic Discharge Networks Lead will work closely with healthcare providers, business offices, admission personnel, and social workers to enhance patient experiences and operational efficiency.
Candidates must be strategic, highly driven, and data and outcome-oriented, with a particular aptitude in building processes and managing projects from inception to completion. All employees must be able to thrive in a dynamic, fast-paced startup environment. This position is a full-time, hybrid role (combination of remote and in-person).
The Strategic Discharge Networks Lead will, at a minimum, have the following responsibilities:
Network Management:
- Oversee and expand our Preferred Provider Network, establishing and managing partnerships with LTC facilities.
- Develop and implement strategies to increase referrals and placements within the network, ensuring efficient bed utilization for network partners.
- Ensure facilities meet our quality standards, including clinical fit, geographic proximity, and care quality metrics.
Patient Transition Coordination:
- Coordinate the transition process for patients moving into LTC facilities, working alongside social workers and healthcare providers to streamline documentation and approvals.
- Oversee the utilization of our patient matching algorithm, ensuring patients are matched with facilities based on clinical needs, location, and quality of care.
- Ensure facilities receive standardized clinical and financial documentation (e.g., 3871B, H&P, Med-list, MOLST) to minimize administrative delays and improve transition speed.
Data-Driven Process Optimization:
- Leverage our data analytics tools to provide detailed facility performance insights and patient care quality metrics.
- Continuously analyze referral and placement data to improve transition outcomes and ensure optimal facility utilization within the network.
- Use feedback from facilities, patients, and families to enhance and refine the transition process, ensuring both patient satisfaction and operational efficiency.
Stakeholder Engagement:
- Serve as the primary liaison between LTC facilities, social workers, healthcare providers, and our team.
- Ensure strong collaboration between our company and its partners to provide coordinated patient care and financial support.
- Present quarterly performance reviews and data-driven recommendations to network facilities to drive continuous improvement.
Scaling and Expansion:
- Lead efforts to scale the PPN across new regions, ensuring a smooth onboarding process for new facilities and maintaining high standards of patient care and operational excellence.
- Develop scalable processes to handle increasing referral volumes and ensure high-quality transitions as the network grows.
Key Performance Indicators
Pilot Contract Extensions:
- Achieve at least a 95% extension rate for pilot contracts with customers by demonstrating measurable outcomes and value during the pilot phase.
Zero Issued Credits:
- Maintain a track record of issuing zero credits to customers, reflecting operational excellence and consistent delivery on service agreements without need for compensation or service corrections.
- Attain a Net Promoter Score (NPS) greater than 90% from customer business and admissions offices, as well as leadership, by ensuring high satisfaction levels across all stakeholders involved in the patient transition process.
- A degree or equivalent experience.
- 2-5 years of experience (or equivalent internal experience) in healthcare, with a basic understanding of long-term care, skilled nursing and assisted living transitions.
- Must be comfortable reviewing and analyzing patient records.
- Must be familiar with medical terminology.
- Familiarity with Customer Relations Management (CRM) platforms, proficiency with Microsoft Office Suite and/or Google equivalent products (i.e. Google Docs, Google Sheets, Google Slides)
- Strong understanding of issues and challenges related to healthcare delivery and care options for seniors, including private pay assisted living, long term care, entitlement benefits, and alternative housing and care options.
- Creative ability to assist in the development of new processes and strategies as they relate to the services we offer.
- A belief in communicating openly, listening to others' ideas, and collaborating to arrive at great solutions.
- Strong verbal and written communication skills.
