Care Manager – Senior Client Care Coordination

📍 Peninsula & South Bay, CA | Full-Time | Vehicle Provided

Position Overview:
We are seeking a Care Manager/Care Coordinator to join our team and serve as an advocate for our clients’ care. This role combines organization, empathy, creativity, and problem-solving to ensure each client receives the care, attention, and dignity they deserve.

Approximately 50% of your time will be spent traveling around the Peninsula and South Bay to visit clients in their homes or care facilities (a company vehicle is provided).

Key Responsibilities:

  • Coordinate all aspects of client care, including medical appointments, medication management, and in-home services.
  • Communicate regularly with families, caregivers, and healthcare professionals to ensure continuity and quality of care.
  • Visit clients in their homes or care communities to check on well-being and care quality.
  • Advocate for clients during medical appointments and care meetings.
  • Order and manage delivery of medical supplies, medications, personal items and necessary equipment.
  • Oversee implementation of long-term care benefits and insurance coordination.
  • Creatively enhance each client’s quality of life through personalized engagement and support.
  • Maintain detailed and accurate documentation of all client interactions and care plans.

Qualifications:

  • Background or experience in healthcare, social work, gerontology, nursing, or a related field strongly preferred.
  • Experience working with older adults, especially those with dementia or cognitive decline, is highly valued.
  • Strong communication, organizational, and advocacy skills.
  • Compassionate, patient, and dedicated to improving clients’ lives.
  • Ability to travel locally across the Peninsula and South Bay (vehicle provided).
  • Reliable, professional, and able to work independently.
