The Care Transitions Coordinator will join our business development team with a focus on driving business from new referral sources while maintaining relationships with our existing customers. This role reports to the Business Development Director, and will be expected to build and execute on data-driven processes and strategies that continually elevate SilverStay’s level of service and encourage growth and expansion.
Candidates must be energetic, highly driven, data and outcome-oriented, and able to thrive in a dynamic, fast-paced startup environment. This position is a full-time role and is focused on the Maryland market. Remote working is allowed.
The Care Transitions Coordinator will at a minimum have the following responsibilities:
- Develop and execute tactics to maximize referral growth and placement of patients.
- Maintain regular contact with potential referral sources including hospitals, assisted living facilities, skilled nursing facilities, physicians, home health and hospice agencies.
- Build relationships with local community agencies to encourage word-of-mouth referral activity.
- Assist team with cold calls, organizing schedules, scheduling client tours, choreographing client logistics, and collaborating with Transition Specialists to advance cases efficiently and safely. .
- Conduct industry analysis and local market analysis in order to maintain a full picture of our opportunities and competitive landscape.
- Ensure company clinical and financial approval processes are executed in a timely manner.
- Participate in contract closings and price negotiations with contract companies, as assigned.
- Other duties as collaboratively developed.
Key Performance Indicators:
- Development of impactful referral sources that generate meaningful revenue month over month.
- Net Promoter Score (NPS) >90% from Health System representatives, referral sources and the SilverStay Transitions team
- Demonstrates personal and professional integrity in relationships with colleagues, clients and community partners.
- Demonstrates and maintains compliance with clients’ privacy, confidentiality, and HIPAA regulations.
Desired Skills and Experience:
- A degree or equivalent experience.
- At least 3 years of experience in healthcare business development, with a basic understanding of business development and marketing processes.
- Hospital, skilled nursing or assisted living experience preferred.
- Must be familiar with medical terminology.
- Familiarity with Customer Relations Management (CRM) platforms, proficiency with Microsoft Office Suite and/or Google equivalent products (i.e. Google Docs, Google Sheets, Google Slides)
- Strong understanding of issues and challenges related to healthcare delivery and care options for seniors, including private pay assisted living, long term care, entitlement benefits, and alternative housing and care options
- Creative ability to assist in the development of new processes and strategies as they relate to the services we offer.
- A belief in communicating openly, listening to others' ideas, and collaborating to arrive at great solutions.
- Strong verbal and written communication skills.
- Long term desire to play a key role with an innovative company reshaping its industry.
- Participation in ongoing business development and marketing education and training.
