Case Manager

The Mom Project is excited to partner with our client, a leading provider of residential care services, located in Suffolk County, Long Island, NY! They are looking for a Case Manager to provide rehabilitation services, ensure the implementation of medication policies and procedures, and maintain ongoing records in compliance with OMH and Medicaid regulations. The ideal candidate should be compassionate, detail-oriented, and have a strong commitment to supporting residents in their journey towards independence. Our client is willing to train the right candidate, and while previous experience in a similar role is a bonus, it is not a requirement. The ideal candidate will be local to Suffolk County.

Our client offers varied shifts and schedules within a 12-24 hour and 7-day week span, allowing for great flexibility in shift selection. Both full-time and part-time positions are available.

Core responsibilities:

1. Provide rehabilitation services including Assertiveness/Self-Advocacy Training, Community Integration/Resource Development, Daily Living Skills Training, Health Services, Medication Management and Training, Rehabilitation Counseling, Skill Development Services, Socialization, Substance Abuse Prevention Services, and Symptom Management
2. Responsible for implementation of Medication Policies and Procedures
3. Responsible for reporting, maintaining, reviewing, and regularly updating ongoing records according to Documentation Guidelines and for compliance with OMH and Medicaid regulations and Agency Policies and Procedures
4. Assist with the development and implementation of Individual Service Plan goals
5. Assist residents in establishing linkages with mental health/health clinics, vocational, educational, financial, social, substance abuse prevention, and recreational resources as needed
6. Provide transportation for residents to day programs, appointments, and other program-related activities using personal vehicle if agency vehicle is unavailable
7. Attend and participate in staff meetings, training sessions, ISP Meetings, Quality Assurance Audits, including Utilization Review Meetings, and other relevant meetings as scheduled

Qualifications/Skills:

1. Strong interpersonal and communication skills
2. Ability to work independently and as part of a team
3. Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint)
4. Ability to maintain confidentiality and adhere to agency policies, including Code of Conduct and Confidentiality Policies, Medicaid Compliance Program and Protocols, and Justice Center Code of Conduct
5. Must be ambulatory and present, able to perform all job functions without limitations, up to and including life preservation activities
6. Reliable transportation and valid driver's license required
7. Local to Suffolk County, Long Island, NY
8. Previous experience in a similar role is a plus, but not required, as our client is willing to train the right candidate
