Job description
Our Client is looking for a Patient Advocate to join their team servicing Chicago and North Chicago area ASAP!
This is a part-time, hourly role $60/hr.
*Must be available during the hours of 9A-3P on working days*
Core Responsibilities
- Clinical Advocacy & Care Coordination
- Coordinate care across healthcare systems to ensure continuity and quality.
- Maintain accurate medication lists and client medical profiles.
- Conduct clinical assessments, medication reviews, and home safety evaluations.
- Support hospital discharges and transitions of care.
- Identify and resolve barriers to care.
Client & Family Communication
- Explain provider recommendations clearly and understandably.
- Attend medical appointments and translate medical information.
- Reinforce care plans and promote informed decision-making.
- Support client autonomy while respecting personal values.
- Collaboration & Documentation
- Liaise with providers, facilities, and community resources.
- Document all activities accurately and timely in electronic record.
- Maintain accurate billing and reporting.
- Ensure HIPAA compliance and strict confidentiality.
- Professional Development
- Maintain active Illinois RN license.
- Maintain or pursue Board Certification in Patient Advocacy.
- Participate in ongoing continuing education.
Qualifications/Skills
- Active Illinois Registered Nurse (RN) license.
- Minimum 3 years experience in geriatrics, care coordination, case management, community nursing, OR chronic disease management.
- Psychiatric experience a plus.
- Strong independent clinical judgment.
- Excellent written and verbal communication skills.
- Reliable transportation for local client visits.
- Must have own transport
